Support module versions and terms of use

Closes #1 (closed) and #2 (closed). The manifest file format now supports "version" and "terms_of_use" fields. If present, these are displayed to the user.

This MR also removes fsl/__init__.py, and fsl/scripts/__init__.py, as fslpy >= 3.6.0 defines them as native namespace packages
