Fixed that funky lookuptable label list creation code, avoiding

situations whereby two lists could be simultaneously added. Also fixed
GLLabel refresh issue (related to previous commit). Also,
LookupTablePanel was re-creating label list even when the lut had not
changed - no longer doing this.
